This survey is intended to help us gauge where we are (City Chapel) and to help the leadership and staff in making decisions going forward for the coming year.

What this survey will look like (What am I getting myself into?):

  • 5 minutes total (For real!)

  • It’s anonymous!! We are looking for honesty and trends so please be honest with us! There is an option to identify yourself at the end but it is not required.

  • The questions are multiple choice or yes/no. There is only ONE fill in the blank and one optional comment box at the end!

This survey will help the leadership in:

  • Prayer — what to pray for in our community

  • Events — where is our community needing/wanting to grow in

  • Preaching — what books of the bible, topics, issues do we desire to learn more about

  • Encouragement — Seeing growth, wins, and health of our community
